As you know, one of the main goals of the MHSA Technology Plan is to increase consumer and family empowerment by providing the tools for secure consumer and family access to health information within a wide variety of public and private settings. To help in the assessment and development of our plan we have developed the attached survey to gain participation from our customers and families.

We will evaluate the results of this survey and will utilize it to help drive our Technology Plan as it relates to client access. With our customers inputs we will better understand the technological needs and barriers facing our client and families as we seek to improve their access to health information.
